We are looking for an experienced Advanced Clinical Practitioner with a strong interest in frailty care to join our established multidisciplinary team delivering care within our community hospitals.
This role combines advanced clinical practice with clinical leadership; you will provide line management and professional supervision for the ACPs working within the service together with leading and coordinating the educational supervision of trainee ACPs.
You will provide advanced clinical care to patients with frailty, applying expert clinical reasoning and collaborative decision-making. You will contribute to the Trust-wide ACP strategy and frailty pathway development alongside supporting business planning, service redesign and efficiency initiatives.

Main duties of the job
The post holder will provide autonomous advanced clinical care to frail older adults admitted to community hospital wards across 6 community hospital sites. They will provide line management, professional leadership and clinical supervision for the Advanced Clinical Practitioners in the Community Hospitals.
The post holder will provide leadership and oversight to ensure Advanced Clinical Practitioners maintain professional standards and appropriate evidence for revalidation in line with national frameworks.
The post holder will lead and coordinate the educational supervision of trainee Advance Clinical Practitioners, supporting staff with educational programmes.
The post holder will lead or support frailty-related audits, service evaluation and quality improvement, using outcome data to improve patient safety and experience.

Person specification Knowledge requirements Essential criteria - Expert-level theoretical and practical knowledge in frailty and complex multi-morbidity
- Highly developed advanced clinical assessment, diagnostic skills and medical management skills
- Advanced skills in advance care planning, escalation planning and end-of-life care
- Ability to prescribe safely and effectively within scope of practice
Desirable criteria - Experience leading resuscitation and medical emergencies
- Specialist frailty service development experience
Qualifications Essential criteria - Registered healthcare professional with the NMC, HCPC or equivalent professional body
- MSc in Advanced Clinical Practice (or e-portfolio) mapped to the four pillars of advanced practice, evidenced by a digital badge
Desirable criteria - Additional postgraduate qualification related to frailty, gerontology, acute medicine, community medicine or leadership
- Teaching / mentoring / supervision qualification
- Leadership or management qualification (e.g. NHS Leadership Academy programmes)
Further Training or Job-Related Aptitude and Skills Essential criteria - Independent Prescriber
- Evidence of ongoing personal development in advanced clinical practice relevant to frailty / older people’s care
- Excellent verbal and written communication skills
Desirable criteria - Experience of regional or Trust-wide ACP role development
- Experience influencing Trust-wide or system-level strategy
- Experience leading or coordinating research, audit or service evaluation
- Experience presenting at regional or national forums. Publication or contribution to best-practice guidance
Experience Essential criteria - Significant experience working as an Advanced Clinical Practitioner at senior level
- Extensive clinical experience in frailty / older people’s medicine / complex long-term conditions Experience of autonomous clinical practice, including assessment, diagnosis, treatment and discharge planning
- Experience of clinical leadership within a multidisciplinary team
- Experience of service development and change management
- Experience of supporting, supervising and appraising ACPs (qualified and/or trainee)
- Experience of contributing to clinical governance, audit, incident investigation and quality improvement
Desirable criteria - Experience working across community hospitals or integrated care settings
- Experience of regional or Trust-wide ACP role development
- Experience representing services at directorate, Trust or regional level
Contractual Essential criteria - Ability to travel and work across multiple community hospital sites with access to their own vehicle
