About this role
Continuity of care is key for all individuals, but especially for those at the end of life. Integrated Care Records (ICR) also known as Electronic Palliative Care Coordination Systems (EPaCCS) aim to support people approaching the end of life to receive consistent care, in line with their wishes, that is coordinated across multiple services.
An end of life integrated care record has been created with the digital team on the platform 'Graphnet'. This post will involve the planning, creation and delivery of awareness and education supporting the implementation of this End of Life Integrated Care Record across the Bath and North East Somerset, Swindon and Wiltshire. This will traverse primary, secondary, community and private/independent services. It will include engagement with clinical and non-clinical environments.
The clinical and non-clinical impact of the process will be measured as part of the project and the post holder will be expected to have a significant role in planning and carrying this out.
Applicants will be self-motivated being able to meet deadlines and engage with, learn from and educate patients, families and staff from different professional disciplines. This is an exciting opportunity to be part of the development and roll out of a regional innovative concept.
The post is part time for 2.5 days a week (0.5wte) for a fixed term contract of 3 years. The role will be supported by the lead palliative medicine consultant, and will involve travelling to a variety of sites with flexibility for aspects to be done from home.
