Job Summary

As a Care Coordinator, you’ll play a vital role in coordinating care services and ensuring high-quality support is delivered to our clients. You’ll work closely with carers, clients and families to keep everything running smoothly.

Duties

  • Liaise with clients, their families, and care providers to assess needs and develop care plans
  • Schedule and coordinate care services, including arranging appointments and visits
  • Monitor and evaluate the effectiveness of care plans
  • Maintain accurate records of client information and care plans
  • Provide support and guidance to care providers
  • Respond to inquiries and resolve issues related to care services

Experience

  • Previous experience in an office environment is essential
  • Excellent communication and interpersonal skills
  • Strong organisational abilities with attention to detail
  • Knowledge of health and social care services is advantageous
