About this role
To work with individuals, families, schools and community services. To work in multi-agency partnerships, for example, Social Care, Education, Police and Voluntary Services. To work inter agency with 0-5yrs Health Visiting, Speech and Language and Primary Care Teams. To work in liaison with University Hospital Plymouth NHS Trust departments, e g Paediatric Outreach, Emergency Accident, Child and Adolescent Mental Health Service. To work with Educational establishments e.g. Plymouth University and Professional Training & Development Department To work in Educational settings, Children Centres, and clinics To work under the supervision of the Practice Accessor/ Mentor participate in the assessment of health needs and formulation of care plans for children, young people, their families and communities, this includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic, school or community setting e.g. parenting skills, public health, group work and child health clinics. In collaboration with the Public Health Nursing team, identifies individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, in particular seeking to identify disadvantaged groups and address health inequity. Offers advice and support to children, young people and adults in accordance with evidence based practice. Under the supervision of the Practice Accessor/ Mentor completes the Family Health Needs Assessment, working with the multi-agency team in supporting children in need and those at risk of significant harm, including observing in case conferences and other meetings and contributing to reports where appropriate. This includes learning about the role of lead professional or key worker for children and their families with the most complex needs.
