About this role
The role will involve conducting physical health and wellbeing assessments and interventions. The efficient and accurate documentation of each assessment and taking any action as appropriate within their scope of practice and within the relevant trust policies and processes for the team.To effectively undertake the role, duties will include:1. Arranging physical health appointments in line with trust policy. 2. Developing and reviewing appropriate physical health and wellbeing care plans identifying, organising and signposting to other resources as appropriate. 3. Providing defined assessments/ interventions/ activities as part of the physical health appointment within your scope of practice and level or responsibility. 4. Completing relevant paper and electronic documentation, reports and letters in line with the associate practitioner role and local processes, in a timely and effective manner. 5.Provide information, advice and signposting to promote good physical health and wellbeing e.g. Smoking cessation, healthy diet, exercise, alcohol consumption, employment support etc.
