CAMHS Learning Disability School Liaison Practitioner

About this role

To work as the identified CAMHS Schools Liaison Practitioner to deliver early help consultation, advice and information sharing to community services and schools working children and young people who may present with moderate to severe learning difficulties and emerging mental health difficulties. Co-ordination and implementation of evidence based targeted interventions in a timely manner, in collaboration with the other clinicians in the Neurodiversity pathway. To co-ordinate and contribute to the early assessment to ensure that children and young people access timely assessments and the appropriate targeted interventions to the current developing difficulty. Organisation and delivery of group based needs let groups within the school and community setting. To support the delivery of evidence based parenting programmes for children and young people with mental health issues. This role includes case holding responsbility. This will require the post holder to take a lead in representing CAMHS in multi-agency meetings such as EHAT, EHCP, CIN meetings or child protection meetings to co-ordinate and have oversight of mental health element of a multi-agency plan around a child. To function as a member of the Neurodiversity pathway taking appropriate responsibility for the implementation of decisions made by the Pathway. Ensure planned interventions are implemented effectively and evaluated using Routine Outcome Monitoring (ROMS) and minimum data set. Participate in the delivery of training programmes for schools and community services from a portfolio of available CAMHS training Maintain standards of care to highest level within available resources. To actively participate in line management with senior and junior members of staff, team meetings and case discussions. Post holder is responsinle for attending clinical supervision a minimum requirement of an hour per month. The Post holder will need to oragnise their own workload under the support and supervision of the clinical pathway manager. To liaise effectively with the other CAMHS pathways across the city. To develop and maintain relationships with providers statutory and non-statutory. To maintain confidentiality in respect of the young people at all times. Communicate clearly with children and young people, their families and all other health professional groups about their health problems. Actively participate in clinical governance and quality improvement initiatives, e.g. clinical audit, essence of care or clinical policy development. Ensure that children and young people are encouraged to play a part in decision making about the help they need and how they can access specific interventions. This is to promote person-centred planning.
