About this role
To enact the professional and legal responsibilities for all aspects of the post-holders own clinical practice. 2.To work in conjunction with the OUH ward therapy teams to carry out comprehensive assessments of patientsand form specialist treatment and rehabilitation plans in partnership with the patient, their carers/family and the multi-disciplinary team. 3.To interpret and analyse multiple sources of highly complex clinical information to form accurate therapy diagnoses in the physical management of patients. 4.To formulate and deliver comprehensive therapeutic interventions alongside the ward therapy teams, based on expert clinical skills, evidence-based practice, and patient preferences. 5.To work with the hyperacute rehabilitation team and ward therapy teams to establish and support appropriate discharge planning and onward referrals. 6.To work with the ward therapy teams to evaluate patient progress against agreed treatment goals and be able to use advanced clinical reasoning skills to modify plans accordingly.To act as a patient advocate negotiating changes to clinical treatment plans as appropriate. 7.To determine a patients mental capacity to understand and to gain informed consent for treatment. To work within a legal framework with patients who lack the capacity to consent and are deprived of their liberty. 8.To co-ordinate intervention where there is a need to collaborate across multiple disciplines and provide advice or education as appropriate. 9.To maintain accurate written records using an electronic patient record system and maintain fidelity to professional standards. 10.To determine and mitigate for risk of patient and staff harm using the agreed risk assessment frameworks and the Trusts manual handling policy and to ensure that team processes are designed to mitigate clinical risk. 11.To demonstrate physical ability to carry out therapy assessment and interventions including manual therapy techniques and therapeutic handling 12.To demonstrate highly developed dexterity, co-ordination and palpatory skills for assessment and manual treatment of patients 13.To represent thehyperacute rehabilitation teamand/or individual patients at multi-disciplinary team meetings as required, to ensure delivery of co-ordinated care.
