Case Manager RAAR

About this role

Ensure that case management is delivered with the patient at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual. Make decisions regarding complex case management issues in line with relevant guidance and with support of senior colleagues where needed. Oversee and advise on the safe and appropriate transition of complex patients to their next placement on a rehabilitation trajectory to ensure flow through RAAR. The post holder will be required to regularly review individuals and may be required to assist with collating comprehensive reports providing assurances to relevant stakeholders such as the ICB, other rehabilitation providers, social services, housing partners that plans are appropriate. Initiates and participates in evidence-based projects and is responsible for implementing changes within designated team/area and setting and monitoring standards of practice. Facilitate with processes such as D2A, onward referrals, equipment provision, in order to ensure timely discharge from the RAAR service.
