About this role
This post will involve working in outpatient clinics providing both rapid access, admission avoidance clinics for the frail population of Hillingdon and general elderly medicine clinics.
The current set up of this post is for 2 full days of clinics and SPA worked flexibly so will suit someone looking for less than full time or flexible working. Our Rapid Access Clinics for the Elderly (RACE) run up to 5 times a week providing rapid assessments of community dwelling frail patients referred by their GP and also follow up for patients seen by our service on the wards or in our Acute Frailty Unit supporting ED.
General clinics serve a wide range of presentations in over 65’s where a holistic approach to care is required. We have recently expanded our acute frailty provision and provide 7 day a week frailty SDEC services. Joining our Hillingdon Team provides a unique opportunity for joint working across boundaries as we are an established stakeholder in Hillingdon Health and Care Partners (HHCP).
The partners are the Acute Trust, Central and North West London Foundation Trust (who provide community and mental health services), Hillingdon GP confederation and the Voluntary Sector (H4All) who work closely with the local authority and Brunel University. This is in line with North West London integrated care system (ICS) to provide outcome based care. We are further developing specialist services provided by all four partners in HHCP especially in admission prevention, falls, fracture liaison service, early supported discharge, care home liaison with a new focus on increased community support.
The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.

provide high quality, safe and compassionate care, while drive forward the building of the new Hillingdon Hospital.
