JOB PURPOSE Responsible for the development and delivery of high quality, evidence-based Physiotherapy services across all hospice settings, including the Inpatient Unit, Community Services, and Living Well Centre programme Functioning as a highly-skilled autonomous practitioner, will manage their own caseload and provide specialist programmes, utilising excellent problem solving and clinical reasoning skills. Act as an expert palliative care resource on physiotherapy to staff both within the organisation, and our wider professional partnerships. Promote a strong emphasis on rehabilitative palliative care, providing symptom control, psychological support, rehabilitation and a proactive approach to enablement JOB SCOPE Clinical Provide Specialist Palliative Care services for patients and carers People Responsible for day to day supervision of team members including Staff and Volunteers Financial Assist in monitoring departmental budgets KEY ACCOUNTABILITIES & RESPONSIBILITIES Be professionally accountable for the maintenance of standards and professional practice as set by Hospice Policies/Procedures, the Health and Care Professions Council Standards of Conduct, Performance, and Ethics, and all relevant professional guidelines. Delivers a high-quality Palliative Care service to patients and their carers/families, ensuring that care is planned following individual assessments and implemented and evaluated regularly Work collaboratively with the patient, their relatives and carers to support patients to achieve their personal priorities Engage in and facilitate advance care planning discussions where appropriate, as part of the wider multi-disciplinary team. Liaises with all members of the multidisciplinary team to ensure smooth working across teams to maintain effective service provision Provides continuity and support with group work and promotes feedback from service users. Encouraged all to identify a positive approach and culture towards rehabilitative palliative care. Ensures the maintenance of a supportive atmosphere maintaining good morale Works closely with members of senior staff group to maintain effective communication Liaises with colleagues in other organisations as required. Deputises as team leader to ensure patient needs are met and a safe environment is maintained. Reports on complaints, untoward incidents and faulty equipment to Line Manager and/ Director of Nursing. Maintain a cost-effective service within agreed budget. I Identifies and proposes effective policies and working practices within the team ensuring these are clearly understood when implemented. Reviews work performance of others and undertake appraisals of Assistants Supports research-based practice and the maintenance of a supportive learning environment. Carries out training and competency-based assessments. Participates in Hospice Audit Programme, attends ward and clinical meetings as required Membership of Professional Organisation and practices in accordance with professional code of practice Allocates work to the Physiotherapy/Occupational Therapy Assistant in liaison with the Occupational Therapist. Ensures continuity of service through appropriate delegation to external rehab teams.
