Senior Nurse

About this role

Clinical Responsibilities Undertake advanced clinical assessments, history taking, and physical examinations within the community setting. Develop, implement, and evaluate individualised care plans for patients with complex health needs. Provide care and support for frail patients, including anticipatory and proactive care management. Deliver high-quality palliative and end of life care in line with patient wishes and best practice guidance. Identify deteriorating patients and initiate appropriate interventions and escalation where required. Support admission avoidance and facilitate early supported discharge where appropriate. Work autonomously within professional scope of practice and clinical competence. Maintain accurate and timely clinical records in accordance with organisational and professional standards. Leadership and Team Working Work collaboratively within the multidisciplinary team including GPs, community nurses, allied health professionals, social care, hospices, and voluntary services. Provide clinical leadership and support to junior staff where appropriate. Contribute to service improvement initiatives and integrated working across the PCN. Participate in team meetings, case discussions, and clinical supervision. Communication Communicate effectively and sensitively with patients, relatives, carers, and professionals. Support difficult conversations relating to frailty, deterioration, advance care planning, and end of life care. Professional Responsibilities Maintain professional registration and comply with the NMC Code of Conduct. Participate in continuing professional development and mandatory training. Ensure safeguarding responsibilities are upheld for vulnerable adults
