About this role
We have an exciting opportunity for aBand 6 Dietitianto join our community team, coveringNutrition Support and Gastroenterology. This is a varied and rewarding role for an enthusiastic, self-motivated dietitian interested in developing their skills and knowledge in these specialist areas.
The gastroenterology role focuses on supporting people withIBSandCoeliac Disease, while the nutrition support role helps people at risk ofmalnutrition, combining afood-first approachwith the appropriate use of oral nutritional supplements. You will work collaboratively withmultidisciplinary teams (MDTs), service users, families, carers and care agencies.
Our team works predominantly in ahybrid model, with travel acrossHammersmith & Fulham, Westminster and Kensington & Chelsea. The team meetsface-to-face once a monthandvirtually each weekto support communication, collaboration, team building and staff wellbeing.
We deliver three core workstreams
Nutrition Support, Gastroenterology and Home Enteral Feeding. We are asupportive, innovative teamcommitted to deliveringhigh-quality, evidence-based dietetic care, continuously improving services and prioritising staff health and wellbeing.

- Provide dieteticassessments and treatment plansfor service users via:
- • Face-to-face consultations at health/GP centres
- Telephone and video consultations
- Domiciliary visits
- Manage acaseloadof people with Coeliac Disease, IBS, and those at risk of malnutrition, in line with service standards and under supervision from the Lead Gastroenterology Dietitian
- Organise and manage your ownclinical workload, including administrative tasks, onward referrals, and liaison with MDTs and other services
- Demonstrateflexibility and teamwork, adapting to service needs and direction from your line manager
- Accuratelyrecord clinical activityin the reporting database and generate regular reports
- Build and maintain strong networks withGPs, district nurses, care agencies, primary care teams, and other services to facilitate appropriate referrals
- Collaborate with healthcare professionals acrossprimary and secondary careto ensure coordinated, timely, and person-centred care
- Delivereducation and training sessionsfor internal and external stakeholders, as required by the service.
