To support the service, primary care, the acute sector and community services in providing case management for people with diabetes who are at risk of unplanned admission and facilitate timely discharge from hospital To provide advice and support to social services and residential & nursing homes. To reduce waiting times and contribute to meeting the 4-week Referral to treat pathway
