Stroke Unit Associate Practitioner

Deliver rehabilitation and treatment programs across multi disciplines independently; this may include appropriate and safe use of therapeutic handling, use of medical devices and technology, exercise prescription, group sessions. Recognise deterioration and improvement in patients. Communicating this to relevant MDT members in a timely way. Able to progress and regress treatments, responding to how patient's present each session. Independently be able to use outcome measures and goal setting with patients. Physical ability to carry out interventions. There is frequent requirement to exert moderate physical effort for several periods during the day. Proactive in the discharge planning process, liaising with all relevant parties in a timely manner and making onward referrals as appropriate. Participate in a 7 day working rota.
