Practice Nurse

Deliver routine nursing care including wound care, ECGs, and health checks Lead and support the management of long-term conditions e.g. diabetes, COPD, asthma Deliver immunisations, vaccinations and cervical screening programmes Provide health promotion and prevention advice to patients Support infection prevention and control IPC standards Maintain accurate clinical records using appropriate coding systems Participate in clinical audits, quality improvement, and service development Work collaboratively within the multidisciplinary team MDT and wider PCN Recognise clinical boundaries and refer appropriately to GPs or other professionals Support the delivery of QOF targets and enhanced services
