Job Summary

TheAdultPreoperativeAssessmentDepartmentprovidesaserviceforalladultelectivesurgical patientsattendingChelsea and WestminsterHospital West Mid site,aswellas patientsseenatotherhospitalswhoaretohavesurgeryhere.

PatientsarereferredfromallareasoftheUK andabroad.

TheteamcomprisesspecialistNurses, ConsultantAnaesthetists, HealthCareAssistants.

Tofunctioneffectively,theteam membersrequireclinical informationaboutpatientsthatisnotalwaysreadilyavailableinthehospitalspatientnotes.Thisinformationisvital forappropriateassessmentofriskandtoallowpatientstoproceedtosurgeryas safelyaspossible.

Obtainingthisinformationina timelymanneristhecorefunctionoftherole.

  • · ThepostholderwillberequiredtopromotebestpracticeaccordingtotheTrust’sAccessPolicyandwillbeexpectedtopromoteandutiliseallrelevantTrustPoliciesrelatedtothefulfilmentoftherole.
  • · Whererequired,toliaisewithGPpractices,otherhospitalsandotherdepartmentstoobtainmedicalrecordsandpatientinformationtoensurePOAcansafelyclearthepatientforsurgery,orensure theyaretreatedpriortobeingcleared.
  • · TooverseetheadministrativeaspectsoftheclinicsupportingtheleadforPre-assessment.
  • · Ensurethatallrequiredinformationincludingmedicalrecordsarereadyforrelevantclinic runningthatday
  • · Ensuringthatallpatient requirementshavebeencateredfor(e.g. transport, translatorsetc.)
  • · Takingresponsibilityforplanningandpreparationforclinicsinadvancetoensuresmooth runningon adaily basisandexcellentpatient experience.
  • · Proactivelymanagingpatientpathwaystoensurethat no patients‘fallthroughthenet’.
  • · Actingasapointofcontactforpatientswhoareawaitinginvestigationsarisingfromtheir initial preoperativeassessment.

· ToberesponsibleforescalatingrelevantissuestoASM/LeadNurse.

· Thepostholderwillwork with admin team in POA for booking and Rota.

· ToensurethatappointmentavailabilityismaintainedforHighRiskAssessmentClinic patientsandtoliaisewiththeanaestheticcoordinatortoensureadequateanaestheticcover

· WorkingwiththeleadforPre-operativeassessment, andotherTrust Improvement Teamsinallaspectsofimprovingoutpatientflowsandpathways.

· Attendingallinternal teammeetings, takingminutesand distributingappropriately.

· Cover reception duties

· Using CCS and Cerner for POA

· SupportingtheNursesbyattendingtheregularreviewsessionswithConsultant

· AnaesthetistsandactingonoutcomesonbehalfoftheNursewhenrequired.

· Maintainingestablisheddatacollectionprocessesandrefiningandimprovingasandwhen required.

· EnsuringthatappointmentavailabilityismaintainedinlinewiththedepartmentSLAandthat allclinicsareexpandedorblockedasrequired, escalatingtoLeadPOAC ifrequired.

· ProcuringsuppliestosupporttheadministrativeaspectsoftheclinicviathePECOSsystem

ManagementDuties

· Help manage the band 3 in POA admin

Additional responsibilities

· Promotingtheacceptanceand adherencetotheDataProtectionAct withintheteam.

· Undertakeotherdutiesthatareconsideredtobewithinthegeneral scopeofthepost, as requiredbytheLeadPOA and Scheduling department.
