The All Age Continuing Care service leads the assessment, case management and oversight of the NHS National Framework forContinuing Healthcare(CHC) andFunded Nursing Care(FNC) across Kent and Medway. We support individuals with complex or long‑term health needs by developing, commissioning and reviewing personalised care packages in both community and care‑home settings.
Working as part of a truly integrated system, our teams collaborate closely with the Local Authority, Primary Care Networks and wider partners to deliver safe, person‑centred care. As a Band 7 Senior Nurse Assessor, you will provide expert leadership within the All Age Continuing Care service.
Key responsibilities include
- Leading the CHC Nurse Assessor teamthrough day‑to‑day supervision, mentoring and clinical guidance
- Delivering complex CHC assessments, including Checklists, Fast Track Assessments and Decision Support Tools
- Completing Mental Capacity Assessments, Best Interest Decisions and contributing to Court of Protection/DoLS processes
- Supporting the development of personalised care packages, including Personal Health Budgets
- Ensuring care packages are safe, effective and represent good value for money, while identifying cost‑improvement opportunities
- Working collaboratively with the Local Authoritythroughout the assessment process and supporting the resolution of disputes
- Participating in the Local Resolution Process (appeals), including two‑way discussions and panel involvement
- Providing expert advice on the National Framework, supporting its implementation, training and development across Kent and Medway
- Promoting excellent patient experience, adapting communication with kindness and compassion
- Overseeing team performance, ensuring high‑quality, consistent and timely assessments
