About this role
Due to investment, we have a new role for a band 6 nurse within the Croydon Community stroke rehab team. The post holderwill work collaboratively with the other professionals within the well established MDT, enabling people who have had a Stroke to maximise their independence in the Community. You will be supported by a highly experienced Band 7 nurse and provide nursing input to patients, especially with regards to blood pressure management,medication compliance, continence and skin integrity.
The team work Monday to Friday.
A full valid UK drivers license and vehicle for work are essential for this post.

Demonstrate clinical expertise by delivering hands on care, using highly developed physical, analytical and problem-solving skills, in line with research and evidence based practice.
The post holder will liaise with the nursing staff on the Acute Stroke Unit at Croydon University Hospital and the local hyper acute stroke units to ensure appropriate and timely transfer of care into the Community.
The post holder will be expected to work unsupervised in a variety of settings including the patient’s own home, Croydon University Hospital and other community settings as required.
To ensure that the care delivered to patients is a result of individual assessment of their need using an appropriate assessment framework and actively seek consent in the delivery of such care.
In conjunction with the MDT, be responsible for maintaining excellent standards of care for patients with multiple needs who require complex case management
