Community Psychiatric Nurse

The post holder will

Deliver specialist eating disorder interventions within service users' homes, community settings and outpatient environments. Undertake comprehensive biopsychosocial, risk and physical health assessments. Coordinate and manage a caseload of service users receiving Enhanced Treatment Team support. Provide therapeutic meal support and contribute to nutritional rehabilitation plans. Support admission avoidance and facilitate safe discharge from inpatient services. Monitor physical health, including weight, observations and venepuncture/phlebotomy following appropriate training and competency assessment. Develop and implement person-centred care plans in partnership with service users, carers and the multidisciplinary team. Deliver recovery-focused interventions that promote independence, wellbeing and social inclusion. Provide leadership, supervision and support to junior staff and students. Participate in service development, audit, quality improvement and clinical governance activities.
