Following the successful RCOT pilot, you will help individuals manage common mental health conditions to prevent long-term sickness, making a real difference in their recovery journey

This role provides specialist mental health support within a vocational rehabilitation framework, working closely with primary care and community services supporting patients in our ground-breaking Trailblazer 'Life Re-worked' Programme.

We are seeking a qualified and experienced occupational therapist to work 0.4wte alongside GP practices to provide vocational rehabilitation advice and signposting, assessment and intervention to support individuals to return to or maintain their work role; and advice and signposting for those wishing to return to employment.

The team will support the following - physiotherapy, occupational therapists, psychologist, mental health practitioners and assistant practitioners.

  • To work at an advanced level within the four pillars of advanced clinical practice: Clinical practice, leadership and management, education and research.
  • To take responsibility for developing and shaping the role to meet the needs of the local population in consultation with the PCN leads
  • To autonomously utilise advanced clinical practice knowledge and skills to provide specialist vocational assessment and rehabilitation for patients with common mental health disorders and musculo-skeletal problems and occupational therapy assessment and intervention to the Primary Care Network (PCN) agreed population.
  • To evidence high levels of problem solving, clinical reasoning and exercise independence of judgement to assess treat and manage own caseload of vocational rehab within the PCN.
  • To develop processes to optimise service flow, outcome measurement and present data on the developing role within the PCN.
  • To work as part of a multi-disciplinary team in a patient -facing role, using their expert knowledge of the impact of physical, social, psychological, and environmental and mental health issues on functional independence and vocation impacting an individual’s ability to maintain work, and independence
