About this role
To assess, diagnose and develop individual care/treatment plans for a range of conditions, some of which will be highly complex or will be for patients with complex clinical and social needs. To be responsible for reassessing patients care as treatment progress and alter treatment programme as appropriate. To undertake highly specialist assessment and treatment of patients who may present with long term multiple conditions or with complex presentations of fatigue, brain fog, low mood. To be able to prioritise on needs-based care approach. To demonstrate excellent communication within MDT and other stakeholders. To participate in local and cross borough Inservice training, deliver training and promote the service To keep accurate up-to-date records on patient care and equipment provision, which comply with quality standards of Trust/Professional/Service policies, meet legal requirements and that can be shared with colleagues and carers when appropriate.
