Care Coordinator

Main Duties Coordinate care for patients with long-term conditions, frailty, and complex health needs. Support patients to access appropriate health and community services. Liaise with hospitals, community teams, social care, care homes, and voluntary organisations. Assist with monitoring and tracking patient pathways and outcomes. Support multidisciplinary team meetings and follow up agreed actions. Help identify patients who may benefit from additional support or targeted interventions. Maintain accurate and up-to-date patient records. Provide excellent patient service through telephone and written communication.
