About this role

The Clinical Pharmacist will work as part of the multidisciplinary primary care team, providing expert support on medicines optimisation, prescribing and patient care.

Key responsibilities include

Conduct structured medication reviews and support patients with long-term conditions. Review and optimise prescribing to ensure medicines are safe, effective and cost-effective. Identify and manage medication-related risks, interactions and side effects. Support the management of chronic conditions such as diabetes, hypertension, cardiovascular disease and respiratory conditions. Provide advice to GPs and other clinicians on prescribing and medicines management. Respond to medication queries from patients and healthcare professionals. Undertake medication reconciliation following hospital discharge and care transitions. Support audits, clinical reviews, prescribing quality improvement and QOF-related work. Promote safe prescribing and adherence to local and national prescribing guidelines. Work collaboratively with GPs, nurses, HCAs, reception and other members of the practice team. Maintain accurate clinical records and work within professional, legal and NHS requirements.
