About this role

Frimley Park Hospital Stroke Unit are looking to recruit 2 Stroke Unit Associate Practitioners.

We are delighted to have these new positions to support us in increasing rehabilitation intensity to our stroke inpatients. You will work across the multi-disciplinary team, assisting experienced and specialist registered staff in Physiotherapy, Occupational Therapy, Speech and Language Therapy and Dietitics.

This is a really exciting opportunity to make a real difference in the recovery of our stroke patients.

  • To work under the indirect supervision of registered therapists and clinicians to carry out rehabilitation to stroke patients. This may be working alongside a registered member of staff or independently, completing individual or group sessions.
  • To deliver rehabilitation holistically incorporating multi-disciplinary treatments based on each individuals rehabilitation needs post stroke.
  • To be involved in using outcome measures, goal setting and data collection to demonstrate the effectiveness of rehabilitation in line with current guidelines.
  • To communicate effectively with all relevant parties, including the patient, families, and members of the MDT.
  • To contribute to the discharge planning process in liaison with the MDT.
  • To actively contribute to own CPD and learning. And support others in the team with their learning as appropriate.
  • To work flexibly, including at weekends to help deliver rehabilitation to stroke patients across 7 days.

  • Deliver rehabilitation and treatment programs across multi disciplines independently; this may include appropriate and safe use of therapeutic handling, use of medical devices and technology, exercise prescription, group sessions.
  • Recognise deterioration and improvement in patients. Communicating this to relevant MDT members in a timely way. Able to progress and regress treatments, responding to how patient's present each session.
  • Independently be able to use outcome measures and goal setting with patients.
  • Physical ability to carry out interventions. There is frequent requirement to exert moderate physical effort for several periods during the day.
  • Proactive in the discharge planning process, liaising with all relevant parties in a timely manner and making onward referrals as appropriate.
  • Participate in a 7 day working rota.
