About this role
Work as part of the existing multidisciplinary specialist palliative care team on the inpatient unit at Meadow House Hospice. Take responsibility regarding inpatient referrals of patients to the service in conjunction with the multidisciplinary team. Lead ward rounds, patient reviews and discussions primarily on the inpatient unit. Act as a resource and support to all members of the multidisciplinary team involved in patient care, focussing on collaboration through excellent communication. Ensure that all records are kept in accordance with the Trusts record keeping policy and in line with General Medical Councils Good Medical Practice guidance. Use electronic systems (Cerner and System1) for medical documentation. Provide clinical supervision for trainees working on the inpatient unit and will liaise closely with the Educational and Clinical Supervisors of these colleagues, with the aim of assuming these responsibilities. Support other members of the specialist palliative care team in managing patients with complex needs and in handling difficult consultations. Take part in the appropriate referral of patients to other services, and in handover and sharing of appropriate information with other healthcare professionals, including in house, primary care professionals and colleagues in other hospital trusts. Attend the weekly specialist palliative care MDTs. Be responsible for ensuring that the medical input into that meeting is available, up to date and recorded accurately. Be flexible in substituting for medical colleagues (all grades) to cover both foreseen and unforeseen absences throughout the clinical team at Meadow House Hospice. Provide leadership and support to the multidisciplinary team in community and inpatient settings.
