We are looking fora passionate Community Learning Disability Nurse ready to take their knowledge and skills to the next level, as an exciting opportunity has arisen for a full time permanent position in the Croydon Community Learning Disabilities Team.

The successful candidate will have a strong skillset working with people with learning disabilities and neurodevelopmental conditions, complex physical health and mental health issues.

The successful applicant will join a highly valued nursing service as part of the multidisciplinary teamfor adults with intellectual disabilities, based in the heart of Croydon.

The CLDT comprises Learning Disability Nurses, Psychologists, Speech & Language Therapists, a Dietitian and Physiotherapists. The service is located in Bernard Weatherill House, Croydon Council Headquarter, and has strong working relationships with key partner agencies including Croydon Adult Social Care and Croydon Mental Health in Learning Disabilities (South London and Maudsley).

This is a full time post. However, requests for part-time working and job share may be considered, taking into account at all times the operational needs of the team.

The post holder will provide Community Learning Disability Nursing service to adults with learning disabilities who live in the Croydon area. The post holder will work collaboratively as part of a multi-disciplinary Community Learning Disability health team to identify and address health inequalities for PWLD (People with Learning Disabilities) who live either independently or with families/carers.

Croydon Health Services NHS Trust provide hospital and community services from a number of community and specialist clinics throughout Croydon. The Trust was formed on 1st August 2010 through the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust.
