About this role
The Anticoagulation Specialist Nurse is responsible for delivering safe, effective and patient-centred anticoagulation care through independent clinical practice.
Key responsibilities include
Managing an independent caseload within nurse-led anticoagulation clinics across hospital and community locations. Assessing, monitoring and reviewing patients receiving anticoagulation therapy in accordance with local and national guidance. Providing specialist advice, education and support to patients, carers and healthcare professionals. Interpreting clinical information and laboratory results to support treatment decisions and ongoing management. Maintaining accurate clinical records in accordance with professional and organisational standards. Working collaboratively with multidisciplinary teams to ensure coordinated care and effective patient outcomes. Supporting clinical governance activities including audit, service evaluation and implementation of best practice. Contributing to the development and improvement of anticoagulation services and pathways. Promoting high standards of patient safety, safeguarding, equality, diversity and inclusion. Acting as a clinical role model and supporting colleagues through advice, education and professional leadership. For this role, we are seeking a registered nurse with post-registration experience, excellent communication and organisational skills, the ability to work autonomously, and a commitment to delivering high-quality patient care. Experience in anticoagulation services is desirable but not essential.
