Advanced Clinical Pharmacist - Acute and Emergency

Assist in the delivery of the Pharmacy Strategy, contributing to service development and innovation. Lead on the development, implementation, management, and audit ofmedicines policies and clinical guidelines across the Trust, ensuring safe and effective prescribing for patients in the Acute and Emergency setting. Supervise and support the training of pharmacy staff inpharmacotherapy, including prescribing considerations for conditions for patients in the Acute and Emergency setting. Liaise with healthcare professionals acrossprimary and secondary care to ensure efficient use of medicines and resources, and improve communication of medication related issues. Collaborate with consultants, multidisciplinary teams including specialist nursesto optimise pharmaceutical care and improve patient outcomes.
