Advanced Nurse Practitioner

Key duties include

Providing autonomous assessment and management of patients presenting with undifferentiated and undiagnosed health problems. Taking comprehensive clinical histories, undertaking physical examinations and making appropriate clinical assessments. Formulating differential diagnoses and developing appropriate treatment and management plans. Requesting, interpreting and acting upon appropriate diagnostic investigations and test results within scope of practice. Prescribing medication as an Independent Prescriber, where appropriately qualified, in accordance with current legislation, local formularies and evidence-based guidance. Managing patients presenting with acute illness, minor illness, minor injuries and exacerbations of long-term conditions. Providing appropriate follow-up, safety-netting and onward referral to secondary care, community services or other healthcare professionals where required. Recognising clinical deterioration, red flags and emergency presentations and taking appropriate and timely action. Supporting the management of long-term conditions and complex patients where this falls within the postholder's competencies and experience. Providing health promotion, lifestyle advice, disease prevention and patient education. Working collaboratively with GPs, Practice Nurses, Nurse Associates, HCAs, pharmacists, First Contact Practitioners, social prescribers and other members of the wider multidisciplinary team. Providing clinical advice, support and supervision to less experienced members of the nursing and clinical team where appropriate. Supporting the professional development of the Nurse Associate, HCA and other members of the practice team as required. Participating in clinical meetings, significant event reviews, case discussions and multidisciplinary team meetings. Contributing to clinical audit, quality improvement initiatives, medicines optimisation and improvements in patient safety. Supporting the development and implementation of clinical protocols, pathways and evidence-based practice. Maintaining accurate, comprehensive and timely clinical records using the practice clinical system. Ensuring appropriate coding, documentation and follow-up of patient care. Working in accordance with safeguarding requirements and taking appropriate action where concerns are identified. Ensuring compliance with NHS requirements, practice policies, infection prevention and control, information governance, confidentiality and professional standards. Managing workload effectively, prioritising clinical risk and appropriately escalating or seeking support for cases outside individual competence or scope of practice. Maintaining professional competence through continuing professional development, clinical supervision, reflective practice and appropriate revalidation requirements. Supporting the wider clinical and operational needs of the practice and contributing positively to service development. Qualifications Essential Registered Nurse with current NMC registration. Recognised Advanced Clinical Practice / Advanced Nurse Practitioner qualification, normally at postgraduate or Master's level, or equivalent demonstrable advanced practice competencies. Independent Prescribing qualification with current prescribing competence. Advanced clinical assessment and consultation skills. Evidence of competence in clinical examination, diagnosis and management of patients presenting with undifferentiated conditions. Evidence of ongoing continuing professional development relevant to advanced practice. Sound knowledge of evidence-based practice and current clinical guidelines. Understanding of professional accountability, clinical governance, confidentiality and safeguarding responsibilities. Desirable Previous experience working as an Advanced Nurse Practitioner within General Practice, Primary Care or an Urgent Care setting. Master's degree in Advanced Clinical Practice or equivalent. Experience managing both acute presentations and long-term conditions. Experience of supervising, mentoring or supporting the development of other clinical staff. Experience undertaking clinical audit and quality improvement activity. Experience contributing to the development of clinical pathways, protocols or services. Knowledge and experience of NICE guidance, local prescribing guidance and primary care pathways. Experience using clinical systems such as EMIS. Teaching, mentorship, supervision or coaching qualification or experience. Requirements The successful candidate will be an experienced, approachable and patient-focused Advanced Nurse Practitioner who is able to provide high standards of autonomous clinical care within a busy General Practice environment. They will demonstrate a high level of clinical reasoning and professional judgement and will be confident managing patients presenting with both straightforward and complex healthcare needs. They will recognise the limits of their own competence and seek appropriate clinical advice or escalation when required.
