About this role
Identify, assess, and manage risk, including suicide, safeguarding, and vulnerability. Ensure risk assessments are current and accurately recorded. Work collaboratively within MDTs to support coordinated care delivery. Liaise with GPs, inpatient services, social care, housing, voluntary sector and other agencies. Lead and coordinate safe transfer of care between services. Manage and prioritise a caseload effectively, balancing risk and need.
