Practice Nurse

Your responsibilities will include

Delivering evidence-based nursing care within the practice setting. Managing and supporting patients with long-term conditions including diabetes, asthma, COPD, hypertension, and cardiovascular disease. Undertaking chronic disease reviews. Providing cervical screening, immunisations, vaccinations, travel health advice, and wound care services. Supporting patients with lifestyle changes and delivering health promotion and wellbeing initiatives. Identifying patients at risk of developing long-term conditions and providing proactive interventions. Supporting patients with medication management and self-care. Maintaining accurate clinical records and contributing to quality improvement programmes. Working collaboratively with the wider multidisciplinary team and external healthcare professionals to ensure coordinated patient care. Supporting safeguarding, infection prevention, and clinical governance activities within the practice.
