Senior Clinical Assessor - All Age Continuing Care Hampshire

About this role

The All-Age Continuing Care Team supports over 7,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, Children's Continuing Care and other joint funding and oversight of a budget of 392M. Our roles are community based with flexible working from a local office and home and is a key role to support people to live well with complex health and primary health needs. Post holders will be employed by Hampshire and Isle of Wight Healthcare NHS Foundation Trust, working in partnership with NHS Hampshire and Isle of Wight Integrated Care Board. Our
