Community locations, partner sites and home working 1. Context The NHS Neighbourhood Health Plan sets out a shift toward delivering more care at home or closer to home, creating integrated, community-based services that improve access, experience and outcomes for people of all ages that support people holistically, recognising the interaction between physical health, mental health and social determinants of health. It aims to build healthier, more independent communities by embedding joined up working between the NHS, local government, social care and voluntary partners as the norm rather than the exception. In Sussex we have formed the Neighbourhood Alliance to enable the delivery of this plan. The Sussex Neighbourhood Alliance (NA) forms part of the wider Sussex Provider Collaborative (SPC), which brings together Sussex NHS provider organisations to jointly deliver largescale transformation programmes across the system. The Neighbourhood Alliance is one of the Collaboratives core strategic alliances.
