at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Home First Assessors work closely with people who use our services, their families, friends and carers, who all play aninvaluable role in how our users experience our services.

They are expected to be kind and responsive, professional andinformative and contribute to the quality of the services provided across the Health and Social Care economy.

If you need Skilled Worker visa sponsorship to be able to join us and you are successful in your application, we will reimburse the cost of using the UKVI Priority Visa Service (currently £500), so you are able to join Team KCHFT as quickly as possible.

This financial support is conditional on you successfully completing all stages of the recruitment process, including interview, receipt of a formal offer, and satisfactory pre-employment checks. You need to be willing to pay the priority service fee upfront and, subject to you meeting all other pre-employment requirements and beginning employment with us, you will be able to reclaim this cost through our expenses process.

Please be aware this financial support only applies to the priority service fee. The cost of the skilled worker visa itself will not be reimbursed.

  • Keeping the people who use our services as safe as possible through the use of sound judgement and effective risk assessments, escalating concerns to the relevant professional in a timely manner.
  • Ensuring the people using our services have a good experience by respecting, empowering and working in partnershipwith people throughout the care planning process.
  • Using all resources available, both professional and non-professional that will enhance enablement.
  • Prepare and present cases at a weekly meeting with the Registered Practitioner ensuring that every aspect of the persons journey through the service is available to ensure a judged decision based on up to date information.
  • Carry out initial assessment in a timely way.
  • Support and supervise and guide the Home First Support Workers to enable them to carry out their work confidently and effectively.
  • Carry out risk assessments both for the environment and the person.
  • Be prepared to train as a trained trainer in Moving and Handling.
  • Carry out Moving and Handling assessments once the person is in their own home.
  • Support the person to regain, where possible, their previous levels of independence taking their interests and previousskills, encouraging new skills.
  • Ensuring the best possible outcomes by using up-to-date skills, good and accurate recording skills and adhering to policies and procedures within scope of assessed competence.
