Community Support Worker

About the Role and our Ideal Candidate

Support Workers provide direct, individualised support. As Key Workers, they maintain a person-centred approach, assisting with service planning, with support from management.

Providing direct support to people that use the service in line with each individual's support plan. e.g assisting with personal care, manual handling, medication and eating and drinking etc.

Completing organisational reports and records e.g incident reports,medication charts, seizure charts, activity monitoring sheets,maintain records relating to the support provided to people.

Being a Key Worker for a number of people to help them make sure their service remains focussed on their individual needs and aspirations (individual service outcomes);

Contribute to the development of individual care plans where required, and implement these on an individual basis.

Running and leading session activities both on site at our building base and out in the local community.

Essential Requirements

A relevant qualification, such as an NVQ Level 2 or equivalent or the willingness to successfully complete of this qualification within the specified timeframe

Six months relevant experience, (paid or unpaid) of supporting people with complex needs (i.e. complex health needs and/or low/medium level needs that challenge mainstream services..

Have good written and verbal communication skills including using IT equipment and systems.

Be able to receive and use supervision and appraisals constructively and attend training as required.

The London Borough of Sutton's Support & Inclusion Team is a key part of the adult specialist Community Support Services. Operating from the Sutton Inclusion Centre, a resource hub for adults with learning disabilities and complex needs, the service focuses on promoting independence and helping individuals achieve their personal goals through partnership working.

This is an exciting time of service expansion, and the team is actively seeking new members. Currently, a diverse range of daytime activities is provided from Monday to Friday, including: Art, Music, Sensory sessions, Interactive storytelling, Companion cycling, Skill building, Keep fit, Shopping, Baking, Tac Pac, and Sports and games.
