Are you ready to be the person who can positively affect the outcome of a young persons life? As a Health and Wellbeing Practitioner, youll be right at the heart of delivering impactful, youth-centred support across local schools, youth hubs, and community settings. This role is all about meeting young people where they arewhether thats in the classroom, youth justice system, or local drop-in centreand offering them non-judgemental, holistic help around risky behaviours including substance use and sexual health. Youll hold a caseload of young people and deliver a range of evidence-based interventions, from brief advice to structured psychosocial support. Youll also contribute to health promotion through school sessions, community outreach, and targeted group workempowering young people to make informed choices about their wellbeing. Working closely with our Engagement and Participation team, youll help shape education sessions, peer-led programmes and ensure young peoples voices remain central to service design. Whether youre delivering harm reduction advice, participating in safeguarding meetings, or providing one-to-one care planning, youll play a vital role in reducing risk, building resilience, and improving life chances for children and young people.
