Rehab & Wellbeing Assistant

Supporting rehabilitation and clinical care

Deliver therapeutic interventions (1

1 and group-based) as guided by Physiotherapists and Occupational Therapists Support patients to maximise their quality of life and independence in the areas that matter to them, for example their mobility, daily activities and function Assist with safe transfers, mobility and use of rehabilitation equipment and aids Implement and progress exercise and rehabilitation programmes, with support from therapists, including for complex needs Support people to manage symptoms such as fatigue or breathlessness, helping them stay as active and comfortable as possible. Assist with assessments using delegated tools and outcome measures Monitor and report changes in patients physical, emotional and cognitive wellbeing Work as part of a Multidisciplinary Team and feed back relevant details to appropriate staff Supporting patients on the inpatient unit Work proactively to support and raise the profile of rehabilitation, activity and wellbeing services on the ward

1 and small group sessions at the bedside or in shared spaces Support with personal care where required, maintaining dignity at all times Enable patients to take part in meaningful activities aligned to their goals and preferences Support patients and families with engagement, emotional wellbeing and orientation Escalate concerns appropriately and contribute to handovers Be a point of contact for external providers for wellbeing activities on the ward, such as Pet Therapy or Music Therapy Delivering groups and wellbeing programmes Plan, coordinate and deliver structured therapeutic groups and wellbeing programmes Facilitate groups such as exercise/rehabilitation and symptom management, under the guidance of therapists Design and deliver innovative and inclusive activities that enhance wellbeing Ensure activities are accessible, culturally sensitive and promote connection and identity Contribute to hospice-wide events and engagement initiatives Working with others Communicate effectively with patients, families, carers and colleagues, including in sensitive situations Adapt communication to meet individual needs (e.g. cognitive or sensory impairment) Work collaboratively with the multidisciplinary team and community partners Build links with external organisations and activity providers Support and guide volunteers involved in rehabilitation and wellbeing activities Supporting service delivery Maintain accurate and timely clinical and activity records, including electronic systems Support scheduling of appointments, groups and activities Assist with equipment and resource management Contribute to audit, data collection and service evaluation Support reporting on engagement and outcomes Raise the profile of the rehabilitation and wellbeing service, including sharing stories with patients permission
