We are seeking an experienced and motivated Practice Nurse to join our friendly and supportive clinical team at Riverside Medical Practice. This is an exciting opportunity for a skilled nurse who enjoys working within primary care and is committed to delivering high-quality, patient-centred care. The successful candidate will play a key role in supporting the health and wellbeing of our growing patient population of over 11,500 patients. The Practice Nurse is an integral member of the Primary Healthcare Team and will provide a comprehensive range of nursing services to patients within the Practice. Working closely with GPs, Practice Nurse, Healthcare Assistant, administrative staff and the wider multidisciplinary team, the successful candidate will contribute to the delivery of safe, effective and responsive healthcare services. We are looking for an experienced Practice Nurse who is confident in independently managing chronic disease clinics and undertaking cervical cytology. These are essential requirements for the role. The main duties of the post include the independent management of chronic disease clinics, including diabetes, asthma, COPD and hypertension, ensuring patients receive evidence-based care in accordance with current clinical guidelines. The successful candidate will also undertake cervical cytology, childhood and adult immunisations, travel vaccinations, wound care and dressings, ECGs, NHS Health Checks. The post holder will provide preventative healthcare advice and support patients in making informed lifestyle choices to improve their health and wellbeing. They will work collaboratively with colleagues to deliver high standards of care whilst maintaining a patient-focused approach at all times. The successful candidate will maintain accurate, contemporaneous and confidential patient records using SystmOne and will ensure all clinical documentation is completed in accordance with professional and organisational standards. They will participate in clinical audit, quality improvement initiatives and service development activities where appropriate. The role requires excellent communication and interpersonal skills, with the ability to communicate effectively with patients, carers, colleagues and external healthcare professionals. The successful candidate will be able to work independently whilst also contributing positively as part of a multidisciplinary team. Applicants must be registered with the Nursing and Midwifery Council (NMC) and have previous experience working within General Practice. Experience of independently managing chronic disease clinics and undertaking cervical cytology is essential. Experience of using SystmOne would be advantageous but is not essential. We offer a supportive and friendly working environment within a modern, recently refurbished practice. Full-time hours are preferred, although part-time applications will be considered for the right candidate. Salary is between £18.00 and £25.00 per hour, depending on experience and qualifications. The post holder will be expected to maintain professional registration and participate in continuing professional development, annual appraisal and mandatory training requirements.
