About this role
Main Duties of the Job In this role, you will: Support the Partners in implementing effective medicine management and patient care within the practice/s. Identify areas for improvement and managing change
Deliver patient-facing clinical care
Conduct consultations, prescribe medication, run clinics (especially for long-term conditions), and support vulnerable or complex patients, including home visits.
Enhance patient understanding and outcomes
Educate patients and carers on medication use, improve adherence, and provide ongoing support to build confidence in treatments. Ensure safety and quality of prescribing: Monitor high-risk drugs, handle medication incidents, carry out audits, and align practice with clinical standards (e.g., NICE) to improve safety and performance. Collaborate, train, and develop services: Work within the multidisciplinary team, support staff training, liaise with external stakeholders (e.g., community pharmacists), and contribute to service improvement and professional development.
Improve medicines management systems
Oversee prescribing processes, medication reviews, repeat prescriptions, and ensure safe, effective, and high-quality use of medicines across the practice
