Practice Nurse with Interest in Frailty

The role will include

Chronic disease management Health promotion and preventative care Frailty assessments and care planning Supporting care home residents and housebound patients Medication reviews and monitoring Vaccinations, wound care, and routine nursing duties Working closely with GPs, pharmacists, paramedics, social prescribers, and community teams What We Are Looking For We are seeking a nurse who: Is a registered nurse with current NMC registration Has experience in primary care or community nursing Has an interest in frailty, elderly care, or complex care management Demonstrates excellent communication and teamwork skills Is compassionate, proactive, and patient-centred Ideally has experience with long-term condition management and care planning Additional qualifications in frailty, gerontology, or long-term condition management would be advantageous but are not essential.
