Consultant/Senior GP wsi in Community Medicine and Virtual Ward

The lead consultant, senior GP will also liaise closely with consultant colleagues in acute medicine, respiratory and cardiology who each provide virtual support to their patient groups. The structure of the service will develop further once the successful candidate is in post but is likely to involve a daily MDT, telephone advice, and some domiciliary visiting to more complex patients. The ward will receive step down patients from acute Trusts across NCL including UCLH and Whittington Health acute hospitals as well as step up patients from the urgent community response services in the Borough of Islington and Haringey.
