Specialist Stop Smoking Community Health & Wellbeing Coach

Actively engage communities who experience health inequalities through outreach in community, clinical and non clinical settings Attend community settings such as hostels, severe mental illness SMI services and substance misuse services, to deliver stop smoking interventions (the list is dependent on Haringey Borough demand for their residents) Act as a named link worker between the Stop Smoking Service and wider support services, including NHS Talking Therapies, mental health and substance misuse services, Adult Social Care, homelessness, refugee, asylum seeker and migrant health services, health visiting and school nursing (019 services), and community pharmacies. Deliver stop smoking sessions face to face, remotely by telephone or online via a video platform. Promote the stop smoking service to residents and health and social care professionals to increase uptake and retention Deliver awareness sessions and brief interventions in partner organisations Build and maintain strong relationships with healthcare professionals and partner agencies Attend relevant MDTs and pathway meetings where smoking is a key risk factor Support clear referral pathways across primary care, mental health, substance misuse, social care and voluntary sector services Provide warm handovers and coordinated follow-up to improve engagement and retention Support smoking status reviews and medication considerations with GP practices for prescription only medications
