Are you ready to make a real difference in people’s lives?Our Hospital at Home (H@H) Service is looking for an Occupational Therapist to provide expert, home-based care that helps patients stay independent and recover safely at home.

  • Deliver assessments and interventions across care homes, residential units, and patients’ homes.
  • Work closely with a supportive multi-disciplinary team to reduce hospital admissions andfacilitate early discharges.
  • Supervise and mentor junior staff and students.
  • Prescribe, order, and teach the safe use of equipment.
  • Build strong partnerships with colleagues, patients,families and local agencies.
