About this role
We are seeking an experienced Advanced Clinical Practitioner to join our colorectal and stoma care service as a Team Lead. This is an exciting opportunity to combine expert clinical practice with leadership and service development, supporting the delivery of high-quality, patient-centred care across the colorectal pathway.
The post holder will work autonomously to assess, diagnose and manage patients with colorectal conditions, including those requiring surgical intervention and ongoing stoma care. They will also provide clinical leadership to the multidisciplinary team, supporting workforce development, pathway optimisation and improved patient flow, including delivery of cancer and elective care standards.
The post holder will
- Provide advanced clinical assessment, diagnosis and management for patients with colorectal and stoma care needs
- Manage a defined caseload across inpatient, outpatient and community settings
- Deliver specialist stoma care, including pre-operative assessment, post-operative support and long-term management
- Work autonomously within scope of practice, including requesting and interpreting investigations and prescribing where appropriate
- Support delivery of colorectal pathways, including cancer (2WW) and elective services, improving flow and reducing delays
- Provide clinical leadership and line management support within the colorectal and stoma team
- Work collaboratively with consultants, CNS teams and wider MDT to ensure coordinated, high-quality care
- Contribute to service development, audit and quality improvement initiatives
- Act as a clinical expert resource, supporting education, training and development of staff
