About this role
The successful candidate will work as part of the Cardiac Rehabilitation multidisciplinary team, providing a specialist rehabilitation service to patients with a range of cardiac conditions across the organisation within outpatient clinics, Cardiac Rehabilitation exercise classes and Heart Health education sessions. They will support and advise patients and their families during their pathway through the service.
- To manage own caseload, assess, plan, implement and evaluate interventions using evidence based/best practice, working towards patient centred goals in the context of the speciality and as part of the multi-disciplinary team.
- To be accountable for their own individual decisions and professional actions.
- To participate and flexibly assist senior colleagues in the wider service needs and development.
- Comply with the Chartered Society of Physiotherapy Care Standards of Practice and Health Professionals Council Standards of Performance Conduct and Ethics of all other relevant national and local policies and procedures.
- To assist the Team Lead in the provision of the Cardiac Rehabilitation Service as necessary
