Occupational Therapist

To provide an Occupational Therapy service of the highest possible standard for those referred to the serviceby

Clinical

Assessing patients through virtual or face to face means, independently making autonomous decisions in the clinical assessment of patients with complex health and social care needs. Agreeing goal orientated programmes in conjunction with the client, his/her family and other professionals involved and planning and implementing appropriate intervention. Devising rehabilitation programmes using the most up to date available evidence and guidance to enable clients to develop skills that will improve their performance and independence as well as health and wellbeing. Liaising with other health care professionals, making referrals, making recommendations where necessary to reduce the effect of disability on health and wellbeing. Participate in health promotion/education and give advice as necessary to clients and their carers. To formulate and evaluate individualised management and / or treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options in order to formulate a specialised programme of care e.g. supported Self-management. Monitor clients health and medical status; reporting any changes or deterioration to the MDT involved in the clients care accordingly. To attend MDT meetings to present patients when appropriate and to discuss recommendations identified at the initial assessment. To carry a caseload of patients as an autonomous practitioner, including those with highly complex presentations. To delegate clinical work to the Therapy Assistant, as appropriate in order to meet needs of the patient group.
