Team Manager (Planned Care)

Phoenix Futures is looking to recruit a passionate and solution-focused Planned Care Team Manager at HMP Pentonville. The Role
As Planned Care Team Manager, you will support the service manager in overseeing the effectiveness of the case management system, within the substance use service. You will draw on expertise to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed. You will support and lead Recovery Workers, ensuring they provide initial support, continuity and coordination for people who use our service. You will ensure that individuals are accessing the relevant services available in the prison and that there is a plan in place for each prisoner to support their long term engagement with healthcare and the management of any health condition they might have. You will also support the service manager in having oversight and development of the Planned Care interventions available, such as group work programmes and placements for people in treatment onto the Incentivised Substance Free Living unit (ISFL).
About You
As an individual, we are looking for someone who is self-motivated and a good problem solver who can work on their own initiative and as part of a team. You will need excellent communication skills and the ability to navigate challenging situations with a solution-focused approach. The successful candidate will have experience of line management, safeguarding, and working with vulnerable adults presenting with substance use, alcohol use, and associated treatment needs.

This role will be rota-based, working early and late shifts, Monday to Saturday on a rolling rota basis.
