Forensic Flow and Pathway Manager

Use own initiative, personal experience, and job- related training in deciding on the approach and interventions required when working with a service user in delivering peer support, in line with supervision, mentor and other peer networks The post holder may be required to work in different locations as required by service need. To ensure day to day practice reflects the highest standards of governance, clinical effectiveness, safety, and patient experience. Role may involve visiting a ward or community service as outlined above. The post holder will need to work flexibly. Lead communications and resolution of disputes with external agencies. Facilitate regular internal meetings to share updates, address challenges and promote a culture of continuous improvement. Collaborate with clinical teams, operational leads and other stakeholders to facilitate timely discharge. Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups. Network wit h ICB and Local Authority partners to develop partnerships and strong working relationships, in order to improve patient flow. The post holder should also support senior members of the clinical team to establish working relationships with commissioners and the local authority. Network with NLFC / NHSE to transform flow systems.
