Mental Health Practitioner

Mental Health Practitioner (Nurse, Social Work, Occupational Therapist) Triage; screening of referrals and other correspondence such as Safeguarding Alerts, Police Reports and enquiries from GP's, LBI Housing/Social Care and patient/carer emails SAM oversight of safeguarding referrals - Assessment of current care and support needs - Interventions based on speciality of practice, including joint working with other C&I Services - Engagement in service delivery and development; attendance to MDT meetings, supervision and other training opportunities.

Responsibilities

-Completing thorough assessment and care planning alongside Dialog + - Documenting assessments on EPR systems - including EMIS and Carenotes - Engaging in supervision and supporting supervision of junior staff and students - Maintaining mandatory training and engaging with other learning opportunities
