About this role

Working with the wellbeing service team, you’ll support people to make positive lifestyle changes so that they can improve their health and wellbeing. Based within a person-centred service, you will help individuals identify achievable goals and build the confidence to make lasting changes.

This role combines health improvement, behaviour change and physical activity support. You will work with a wide range of people, including those with long-term health conditions, delivering personalised advice, specialist exercise programmes and ongoing motivation. You’ll also work closely with healthcare professionals, community partners and support services to provide a joined-up approach to improving health outcomes.

What you’ll do

  • Assess clients’ needs and develop personalised action plans to support healthier lifestyles
  • Deliver physical activity sessions, lifestyle interventions and health improvement advice
  • Support and motivate clients to achieve and maintain their health goals
  • Manage a caseload, monitor progress and maintain accurate records
  • Work with healthcare professionals and community organisations to provide coordinated support

What we’re looking for

  • A recognised GP referral qualification or equivalent Level 3 qualification, plus a coaching, teaching or physical activity qualification
  • Experience supporting behaviour change and delivering physical activity programmes
  • Strong communication skills with a supportive, non-judgemental approach
  • Ability to manage your own workload and build positive relationships with a wide range of people
  • Commitment to personal development and delivering inclusive, person-centred support
